Parathyroid carcinoma (PC) is a very rare endocrine cancer with aggressive behavior, high metastatic potential, and poor prognosis. Surgical resection of affected gland(s) other involved structures the elective therapy. Pre-operative intra-operative differential diagnosis benign parathyroid adenoma remains challenge. The lack clear pre-operative does not allow one, in many cases, to choose correct surgical approach malignant PC, increasing persistence, recurrence rate, risk metastases. An initial wrong adenoma, minimally invasive parathyroidectomy, associated over 50% occurrence metastases after surgery. Genetic testing could help identifying patients at congenital PC (i.e., CDC73 gene) driving choice neck surgery extension.
